Off-Site Activity and Transportation Agreement
This Off-Site Permission Agreement supplements and is incorporated into Port Rover’s Client Service Agreement. Both agreements apply whenever your dog participates in transportation or off-site activities. This agreement does not replace or limit any terms of the Client Service Agreement. If the agreements conflict regarding an off-site activity, the terms of this Off-Site Permission Agreement will apply to that activity.

Port Rover, LLC, located at 3112–3114 Tyre Neck Road, Portsmouth, VA 23703, requests your permission to transport your dog and take your dog off-site for activities such as shuttle rides, individual walks, pack walks, field trips, enrichment activities, and other outings offered as part of your dog’s school-day experience.  Dogs will be transported in a vehicle operated by a licensed Port Rover staff member and secured using an appropriate crate, restraint, barrier, or other safety equipment selected by Port Rover.

Required Equipment
To participate, dogs must arrive with properly fitted and well-maintained equipment, including:

  • A harness, martingale collar, or properly fitted prong collar approved by Port Rover
  • A standard 4- to 6-foot leash, slip lead, or other leash approved by Port Rover
  • A collar or harness displaying current identification information

Retractable leashes, choke chains, damaged equipment, and improperly fitted or excessively large or small equipment are not permitted. Port Rover may use its own safety equipment or decline to transport a dog if the equipment provided is considered unsafe.

Owner Representation
The owner confirms that they have disclosed all known medical conditions, physical limitations, allergies, medications, behavioral concerns, bite history, escape tendencies, reactivity, and any other information that could affect the dog’s safe participation. The owner agrees to keep the dog’s vaccinations, identification, and required licenses current.

Acknowledgment and Assumption of Risk
The owner understands that transportation and off-site activities involve inherent risks, including, but not limited to:
  • Vehicle accidents or sudden stops
  • Injury, illness, overheating, or exhaustion
  • Escape or becoming lost
  • Encounters with other animals, wildlife, people, traffic, or environmental hazards
  • Bites, scratches, falls, allergic reactions, or exposure to communicable illnesses
The owner voluntarily accepts these inherent risks and authorizes the dog’s participation.

Group Activities
If the dog participates in pack walks or group outings, the owner understands that dogs may have direct or indirect contact with other dogs. Although Port Rover will take reasonable safety precautions, animal behavior can be unpredictable, and injuries may occur even when dogs are appropriately supervised.

Emergency Veterinary Care
If the dog becomes ill or injured and the owner or emergency contact cannot be reached promptly, the owner authorizes Port Rover to obtain veterinary treatment reasonably considered necessary for the dog’s health and safety.  Responsibility for veterinary expenses will be determined in accordance with applicable Virginia law and the owner’s existing service agreement with Port Rover.

Port Rover’s Discretion
Port Rover may modify, postpone, or cancel an activity because of weather, temperature, the dog’s health or behavior, transportation concerns, staffing, unsafe conditions, or any other safety-related reason.  Port Rover may immediately remove a dog from an activity if continued participation could create a risk to the dog, another animal, a staff member, or the public.

Release of Liability

To the fullest extent permitted by Virginia law, the owner releases Port Rover, LLC and its owners, employees, agents, and contractors from claims arising from the inherent risks of transportation and off-site activities. This release does not apply to liability that cannot legally be waived, including liability resulting from gross negligence, willful misconduct, or a violation of duties imposed by applicable law.

AuthorizationBy signing below, the owner confirms that they:
  • Are the dog’s legal owner or are authorized to provide this consent
  • Have provided complete and accurate health and behavioral information
  • Give Port Rover permission to transport the dog and include the dog in approved off-site activities
  • Have read, understood, and accepted the terms of this agreement

This authorization will remain effective until revoked by the owner in writing.
Written revocation will apply only to activities occurring after Port Rover receives it.
